Below you will find the forms needed for membership, parades or club and unit fundraisers. Simply click on the name of the form and print them off.

• Masonic Petition – Used when petitioning to become a member of one of the Masonic Lodges
• Yelduz Shrine Petition – Used when petitioning to become a member of Yelduz Shriners
• Fundraising Request Form – Used for any fundraiser, fraternal or charitable. This form must be submitted and signed by the Potentate before the fundraiser can begin. You must also submit all follow up information to the recorder once the fundraiser in complete.
• Parade Request Form – Used when your club or unit want to participate in a parade or civic activity. Or if you are holding a non-fundraising event sponsored by your club or unit. This form must be submitted to the office and signed by the Potentate before you can participate in the event.
